Registered Nurses & Licensed Practical Nurses

Who are licensed and skilled will work with the physician on coordinating and implementing individual home care plans.

Physical Therapist

Will evaluate rehabilitation needs and provide therapeutic procedures to restore or improve strength,balance, and coordination of functional abilities.

Occupational Therapist

Will demonstrate you and your family how to impress, restore or develop daily activities affected by your illness.

Speech/Language Pathologists

Can assist you with cognitive speech and swallowing concerns/impairments to help you adapt and restore function.

Home Health Aide

Will provide assistance in personal care, such as bathing, hygiene, dressing, and grooming. They may also assist with meal preparations, ambulation, transfer assist or exercises. These services are provided under the supervision of RN or PT.

Medical Social Worker

Will help patients and family with social , emotional and resource needs. Extracare will also coordinate services of homecare physicians or in securing DME and supplies.
